Toolbox Talk Record Template
v1.2
Purpose. Use this template to plan and record a toolbox talk with a work team.
1.How to use this template
Supervisors deliver a toolbox talk at the start of shift every Monday and after any incident or change to plant or procedure. Choose one topic, keep the talk to ten minutes and record who attended. File the record in the safety folder and note the date in the training matrix.
2.Choosing a topic
Topics come from recent incidents, hazards reported, new chemicals or machines, seasonal risks such as heat, and scheduled refreshers on guarding, lockout and manual handling.
3.Delivering the talk
State why the topic matters, show the correct method at the machine or workstation, ask the team what could go wrong and agree on the actions. Invite questions and record any hazard raised.
4.Recording actions
Any action agreed at the talk must have an owner and a due date. Hazards raised must be entered in the Incident and Hazard Register.
5.Attendance
Every attendee must sign the record. Workers who miss the talk must receive it individually before the end of the week.
